Adds deterministic validation for pull request changelog entries and documents the
L-ignoreexemption.Trusted same-repository PRs receive an Amp-prefilled “Add changelog” link, while fork PRs receive generic guidance without exposing secrets. AI remains advisory and cannot affect the validation result.
