The Monstrous Horror Show is a Survival Horror / Explorer Horror & H-Game made in RPG Maker by kaniheadcrab, with a censored version available on Steam.

The story follows four heroines; the shy, weird loner Akenai Yoko, the sex-obsessed gyaru Ikuha Saffron, the sporty teacher Hatenashi Tachinami, and the mysterious, cool-headed, Justice-minded CEO of HH Corp, Hatsumari Hajime. Through varying circumstances, the four of them are lured to the abandoned Fuon Hospital, which turns out to be not-so-abandoned as it is inhabited by monstrous creates known as Kaijin. Their sadistic creator, the nurse named Yumeno Kanae, has been ensnaring people with the intent of turning them into her test subjects, and now the four heroines must find a way to escape the hospital's horrors — particularly No.01, a hulking masked behemoth of a man who will relentlessly chase them down when he finds them.

Gameplay consists of 2D navigation around the hospital's halls, wherein the chosen heroine will have to collect items, solve puzzles, and avoid traps to progress; items include consumables, key items, and "K-Files" written by Yumeno that contain extra information on the Kaijin. In each chapter, No.01 will stalk you, and periodically appear on the map to chase you; you must either hide, or if he catches you, fight him off and lower his HP so you can temporarily escape. There are also various minigames you will have to play in order to progress.

This game contains examples of:

  • Abandoned Hospital: Fuon Hospital, the game's main setting, is an old abandoned hospital that Dr. Yumeno has turned into her headquarters for her human experimentation. Her creations, the Kaijin, roam the dark halls, and anyone who wanders in does not make it out.
  • Anti-Frustration Features:
    • Losing in a particular section of the game, usually a minigame section, will spawn a mask NPC that will give you some boost to make the game easier if you want.
    • The Return Gem allows you to teleport back to the Hub Level during a Stage, though it erases any story progress you have made; this helps if you want to replay a Stage and collect any items you may have missed the first time around without having to redo the entire Stage to the end.
    • The game has quite a few instances of Hopeless Boss Fight, so to prevent you from wasting any items during them, the game only lets you use skills in those fights.
    • The Kaijin Girls Stage has an option if re-entering it to skip to the ending split if you have all the items for the two additional endings, allowing you to get all endings and their respective K-Files in the same save without having to do the final Stage three times.
  • A Success, by Comparison: At the end of each Stage, ultimately the respective heroine "defeats" No.01 only to fall and get captured anyway; however, since they survived for so long, Yumeno is impressed enough that she chooses to not throw them to No.13, and instead "rewards" them with being made a Kaijin and Sex Slave for No.01. It ends up being not the worst fate compared to assimilation, and lets the heroines launch a full-blown rebellion in the secret final Stage.
  • Big Bad: Director Yumeno Kanae is the one behind the creation of the Kaijin in Fuon Hospital and lured the four heroines to the abandoned Fuon Hospital to make them her next victims.
  • Blackmail: Hatsumari, in one memory, attempts to do this to Hatenashi to get her to help her on her mission to find information on Fuon Hospital and keep her quiet; she strips herself and threatens to tell everyone that Hatenasi molested her. Hatenashi counters by claiming it won't work, as she has no interest in small chests.
  • Bookends: Upon starting a new game, you have the option to play a short prologue where you are a nondescript grey humanoid going through a small hallway and learning the mechanics of the game. In the Golden Ending route, Akenai briefly finds herself traversing that same hallway which turns out to be the mind of that boy, actually No.01/Ichi, and the True Final Boss has you play as Ichi once again, fighting against Yumeno to free himself and his newfound harem from her control.
  • Cerebus Rollercoaster: The game flip-flops between horror and comedy frequently. Generally, the main Stages are serious Survival Horror fare, though comedic scenes become increasingly common as the story progresses. Meanwhile, the Prison Slice-of-life portion between each Stage switches to harem comedy between No.01/Ichi and the heroines.
  • Darkest Hour: After completing all five initial Stages, it seems that all is lost for humanity and Yumeno has achieved ultimate victory; the four heroines, including the HH Corp CEO Hatsumari, have been turned into her Kaijin, she's eliminated any way for HH Corp to retaliate against her, she's prepared to launch the final phase of her experiments, and she then orders No.01 to "dispose" of the heroines for good; you, as No.01, are promptly forced to do so. The end credits having played beforehand seems to drive in that everything the heroines have done has amounted to nothing and the story will ultimately have a Downer Ending. However, a secret final chapter is then promptly unlocked revealing that No.01 intentionally took an unintended interpretation of Yumeno's orders and "disposed" of the heroines by removing their mind-control devices and dumping them in the morgue alive; this allows the heroines to stage one final attempt at rebellion, and this time they can finally succeed.
  • Disc-One Final Dungeon: While the four first Stages can be played in any order, Hatsumari’s Stage is the last one chronologically and the game treats it as the final one; Hatsumari, the justice-loving heroine and Arch-Enemy of Yumeno, breaks into Fuon Hospital and mounts a rescue operation for the other three heroines with their cooperation, which actually seems like it will succeed, and it ends with Hatsumari defeating No.01 for real. Then Yumeno wrecks her and turns it into yet another "Shaggy Dog" Story, unlocking a short Stage from Yumeno’s perspective and what seems to be a final Downer Ending... and then the real final Stage, the Kaijin Girls Stage, is unlocked.
  • Earn Your Bad Ending: Of the three final endings, the standard one N is a Bittersweet Ending where the heroines all escape but have to leave Ichi behind. Ending K, which requires collecting all 12 books and the 4 special items in the final Stage, ends with Akenai fusing with Ichi and No.13 into a massive Transhuman Abomination that starts swallowing the entire city, and the remaining survivors having little hope of stopping her. However, Ending M, which is also unlocked by doing this, is a Golden Ending instead.
  • Earn Your Happy Ending: Each of the four initial Stages ends with the respective heroine captured and turned into a Kaijin by Yumeno, then left to become a Sex Slave, and the Yumeno chapter ends with a Darkest Hour for the heroines. However, it is possible for them to escape, as the hidden final chapter gives them a chance to rebel against Yumeno for good, and doing so leads to both a Bittersweet Ending where they escape at the cost of Ichi’s life, or a Golden Ending where Ichi makes it out alongside them (and brings Yumeno) and they resume their harem life without the confines of the prison.
  • Exact Words: Yumeno ultimately screws herself over by not being careful and specific with her words. She has absolute control over her Kaijin and any order she gives them, their bodies carry out regardless of their will; they literally cannot disobey her even if they wanted to. So when she is done with the four captured heroines, she orders No.01 to “dispose” of them. Since she technically didn’t order them killed outright, No.01 “disposes” of the heroines by removing their mind control devices and throwing them in the morgue, still alive, which allows them to launch a final escape plan that succeeds.
  • Failure Is the Only Option: The four initial Stages have you play as each heroine and attempt to escape Fuon Hospital, but no matter what you do, your heroine is doomed; even if you avoid the Bad Endings and beat No.01 at the end, he recovers and captures the heroine anyway, resulting in Yumeno turning her into a Kaijin and Sex Slave. It's only in the hidden final chapter that this is averted and the heroines can actually escape.
  • First-Episode Twist: The game is initially presented as straightforward Survival Horror fare with four heroines that have their own stories. The very existence of the Prison Slice-of-life section of the game, the Hub Level and a major portion of the gameplay where you play as No.01 romancing the various heroines, is hidden until completing at least one Stage, as it only happens when the "Shaggy Dog" Story nature of the heroine’s Stages is revealed.
  • Hope Spot: Hatsumari’s Stage ends with one; she has turned herself into a Kaijin with Anti-K Cells, gotten the cooperation of the other heroines, and defeats No.01 for real, making it look like they will finally manage to win against Yumeno. Then Yumeno herself puts an end to that; she cannot be hurt faster than she heals, and promptly wrecks Hatsumari, then subjects her to the same Kaijinification as the other heroines, render her stage a Shaggy Dog Sotry like the other three. However, this directly leads to an inversion where the heroines are able to mount a final escape after all.
  • Horror Comedy: While Akenai’s Stage is straightforward horror with her in an Abandoned Hospital pursued by a scary, beefy Malevolent Masked Man, the subsequent chapters mix plenty of comedic scenes with the horror, and the Prison Slice-of-life sections in between the Stages are a goofy (if bizarre) harem/dating sim scenario between the heroines and No.01/Ichi, where even the Big Bad Yumeno acts more like a petulant nuisance than the cruel maniac she is in the main game.
  • Hub Level: The Prison, unlocked after completing one Stage, is the area you return to after completing every Stage, and consists of a hallway with No.17 the shopkeeper, the four cells for each of the heroines, and Yumeno’s office once you have beaten her stage. In it, you play as No.01 and engage in a mini-dating sim scenario with the four captive heroines, Yumeno, and No.17. It’s also the only area of Fuon Hospital that survives the collapse in the Golden Ending, with everyone returning there for a visit every now and then.
  • Interface Spoiler:
    • You can instantly tell if a fight is a Hopeless Boss Fight by the absence of the Possessions option on the battle screen, as you're locked out of using them during those fights to prevent you from wasting them.
    • Once you've completed one of the four Stages, the heroine gets captured anyway and Kaijinified; you are then transported to a prison where the heroine is locked away. The presence of three other empty cells in that same area should clue you in that the other three heroines are doomed to suffer the same fate, so nothing they do in the three subsequent chapters will matter.
  • Late-Arrival Spoiler: You can play the four Stages in any order, but since they take place chronologically according to the positions of the heroines’ portraits on the Stage select, choosing them out of order will reveal what takes place in prior Stages, such as the Kaijin forms of the heroines of earlier Stages.
  • Multiple Endings:
    • The Bad Endings happen if you lose or make a wrong choice at any point, and involve the Stage's heroine being captured and brutalized/violated by a monster, then shipped off to No.13 to be absorbed.
    • Each of the four Stages has its own ending, but it just involves the heroine being captured by Yumeno anyway and being turned into a Kaijin, then sent to become their Sex Slave. After completing all four, a brief Yumeno chapter is unlocked where we see a flashback of the titular Big Bad first operating on Ichi, then the end credits roll and she orders Ichi/No.01 to dispose of the heroines for good.
    • The Kaijin Girls chapter is where the "actual" ending split occurs:
      • Ending N, the Normal ending, has the heroines successfully defeat No.01 for real and escape Fuon Hospital, but they're forced to leave him behind in spite of him once being Hatsumari's friend Ichi; the heroines enter a four-way relationship while wondering if Ichi and Yumeno are still alive somewhere.
      • Ending K, the Kaijin ending, has No.01 and No.13 fuse to become a giant monster and the True Final Boss of the route; ultimately, Akenai senses their loneliness and willingly fuses with them to give them a friend, while the other three are forced to escape without her. Akenai, Ichi, and No.13 promptly become a Transhuman Abomination absorbing the entire city into themselves, while the three surviving heroines and Yumeno are forced to team up to find some way of resolving the situation.
      • Ending M, the Monstrous ending, has Yumeno stop the heroines from escaping with intent to kill them, only for No.01/Ichi to break free from Yumeno's control and fight alongside the heroines to protect them; Yumeno is finally defeated and they all escape, but Ichi goes back to rescue Yumeno. Some time later, the heroines build a memorial for Ichi, only for him and Yumeno to show up, and it ends on a comedic/happy reunion; the epilogue section shows Ichi and the others visiting the prison, having returned to their harem arrangement.
  • Relationship Values: Each heroine in the Prison Slice-of-life has their own Trust meter (though called different things depending on the heroine) indicating how much they have bonded with No.01/Ichi, and it can be raised by giving them items; the special items they like can boost it greatly. No.17 has her own version of this with Phials that, if given, unlock her own bond with you.
  • Romance Sidequest: The Prison Slice-of-life sections allow you as No.01/Ichi to romance each heroine after completing their respective Stage, as they are now Yumeno’s captives and she has ordered Ichi to care for them. They have a Trust meter that you can increase by giving gifts. You can even do the same with Yumeno herself after beating her chapter.
  • Self-Censored Release: The Steam version of The Monstrous Horror Show has most of the sexual content removed from the game.
  • "Shaggy Dog" Story: Most of the game ends up being this, as the first four Stages revolving around each heroine ultimately end with them being captured and turned into Yumeno's Kaijin-ified Sex Slaves even if you reach the Stage end, with the only real "victory" being that the heroines aren't immediately sent to No.13 like they are in the Bad Endings. It's downplayed in that this transformation does end up biting Yumeno in the hidden Kaijin Girls chapter, where the heroines can use their newfound powers to stage a rebellion, but it's not until the last few hours of the game that the heroines can actually obtain a real victory against Yumeno.
  • Shop Fodder: The Phials scattered around the hospital, which can be given to No.17 in the Hub Level and exchanged for quite a bit of Yen. Doing so is also necessary to advance her Romance Sidequest.
  • Story Breadcrumbs: The K-Files scattered around Fuon Hospital contain writings by Yumeno on each of the Kaijin test subjects she has created/experimented on, with some found in each chapter. However, they are imposible for the protagonists to decipher; instead, you need to take them to No.17 in the Prison Slice-of-life section after each Stage and she will translate them for you.
  • Title Drop: The title of the game ends up being what Hatsumari titles her operation in the Kaijin Girls Stage to finally escape Fuon Hospital for good.
  • The Very Definitely Final Dungeon: The hidden ???/Kaijin Girls Stage, unlocked after beating all 5 prior Stages, has you cycle through gameplay with three of the four heroines, journeying through Fuon Hospital to destroy the last defenses Yumeno has for keeping everyone trapped and finally allowing the heroines to escape for good.
  • Warp Whistle: The Return Gem, acquired after reaching the Hub Level, allows the player to return to the Hub during any Stage, bringing any ordinary items they may have acquired with them. However, it also undoes any story progress made in the Stage, so it is mainly useful for returning to completed Stages and collecting any items you might have missed, such as K-Files.
