There are many, I assume, that would like to continue using node-tap, and keep their dependencies updated, but either aren't ready for, or don't wish to subscribe to, 100% code coverage philosophy/ideology. This PR addresses this by "bringing back" minimum code coverage thresholds.

Please let me know if there is any additional work, or changes, and I'll address the feedback promptly.

@isaacs are there any changes you'd like me to make to this? Is there anything else I can do to help move this forward? I appreciate you taking a look.

I took the command line switch descriptions from previous versions of node-tap where min coverage was supported and updated the relevant docs that I could find. Are there any other places or docs that I need to update?

@isaacs are you interested in "passing the torch" to one or more developers to keep this project actively maintained? Or perhaps you're interested in archiving/deprecating the project?
